TEAM MEMBER AGREEMENT 

The Woodlands Diving Academy (TWDA) team membership in an annual commitment, to be paid in twelve (12) monthly installments, due the first of each month, September through August.

  1. I will be billed monthly at the following rate, which is $______ including tax, effective the date of this agreement for Athlete. The amount may be increased from time to time at the sole discretion of Chereches Sports, LLC. 
     
  2. Outgoing high school seniors are responsible for the FULL 12 MONTH INSTALLMENTS through August 31, but may request to be billed for the June, July and August dues in their May billing cycle. FAILURE TO PAY THIS INSTALLMENT WILL RESULT IN INTEREST CHARGED AT 1.5% PER MONTH UNTIL PAID. UNPAID BALANCES WILL BE REPORTED TO A COLLECTION AGENCY AFTER 6 MONTHS OF NON-PAYMENT.
     
  3. Fees returned due to insufficient funds will be charged a $35.00 NSF fee and Athlete will not be allowed to practice/dive until all fees are current.
     
  4. If an Athlete is unable to attend workouts, the installment payment is still due in order to retain full team status. If an Athlete is unable to attend workouts for up to three (3) months, they may request to qualify for Inactive Status, which allows diving dues to be reduced to ½ of the monthly billing amount for up to three (3) months. This status may be granted by Chereches Sports, LLC, based on a case-by-case evaluation. The Athlete will be able to rejoin the team in their regular workout group.
     
  5. If an Athlete chooses to leave the program, 30 day written notice is required. The Athlete may continue to workout with the team during this 30 day period and through the end of the month at the end of the 30 day notice, if they so choose. Payment of all monthly dues through the end of the month at the end of the 30 day period must be paid. FAILURE TO PAY THE FINAL MONTHLY INSTALLMENT WILL RESULT IN INTEREST CHARGED AT 1.5% PER MONTH UNTIL PAID. UNPAID BALANCES WILL BE REPORTED TO A COLLECTION AGENCY AFTER 6 MONTHS OF NON-PAYMENT.
     
  6. If monthly installment payments are not kept current, an Athlete will be suspended from the team and may not rejoin the team until all delinquent charges are paid as well as a $100 registration fee. This is subject to space availability in their specific workout group. An Athlete
    will not be assured of a position in their previous workout group.
     
  7. Chereches Sports, LLC, may impose a finance charge of 1.5% monthly (18% annually) on any amount not paid within 30 days of the due date. Chereches Sports, LLC may charge the amount of the delinquent payment plus any finance charge to the credit card/ACH account identified on the Payment Authorization form. All unpaid/insufficient/declined funds will be turned over to a collection agency after 6 months of non-payment.

I acknowledge and agree to the provisions of this Team Membership Agreement. I understand and will abide by all policies of The Woodlands Diving Academy, Chereches Sports, LLC, its coaches and staff.

All athletes of TWDA are responsible for reading and following the rules and guidelines set forth on the following pages. Athletes and parents understand that any violation or failure to observe policies and rules is subject to immediate suspension and/or dismissal from the team.
